Dear NFP Company Rep,
Thank you for providing a temporary transitional duty position for

As you are aware,

is currently
a Lowe’s
employee and as such Lowe’s will require agreement to the following protocols for all volunteer work performed by [[formData.employeeName]]:
- NFP Company Supervisor agrees to speak to Lowe’s TWS Account Manager on the first day of work to confirm.temporary work restrictions, assigned temporary transitional duty position and arrival for work. It will be NFP Company Supervisor’s responsibility to notify all applicable personnel with NFP Company of [[formData.employeeFirstNamePossesive]] temporary work restrictions and assigned temporary transitional duty position.
- NFP Company Supervisor agrees to provide a weekly time card, documenting hours worked and time taken for lunch and/or medical appointments to the assigned Sedgwick claims examiner by 12 noon each Thursday. The time card should be signed by an authorized representative of NFP Company and [[formData.employeeFirstName]].
- NFP Company Supervisor will provide a verbal update to the assigned Sedgwick claims examiner and/or Lowe’s TWS Account Manager at a minimum of every two weeks or as requested.
- NFP Company Supervisor will advise Lowe’s TWS Account Manager immediately if there are any potential conflicts and/or perceived problems with [[formData.employeeFirstName]] so that we can facilitate ending [[formData.employeeFirstNamePossesive]] position at your site.
- [[formData.employeeFirstName]] will not be assigned a temporary transitional duty position that involves working with cash, children under the age of 18, animals, motor vehicles and/or power equipment.
- Lowe’s and /or its legal representative(s) will be allowed on-site access if requested in writing to conduct an investigation if circumstances warrant.
- Lowe’s and/or its legal representative(s) will be allowed all applicable information and/or documentation related to [[formData.employeeFirstNamePossesive]] volunteer placement at NFP Company as requested by Lowe’s and/or its legal representative(s).
