participating in the Return to Work program. I have attached the documents you will need for
time with you. Below is a quick reminder of the details we discussed and a summary of the [[formData.fourFive]] attached documents.

Attachments:
1. Modified/Light Duty Job Description - provides the physical work restrictions and physical activity guidelines to be followed. Please return on the meeting or start date, with both you and the employee’s signature.
2. Return to Work Program Agreement – please return on the meeting or start date, with both you and the employee’s signature.
3. Time Card – record hours daily and return each week by the deadline at the bottom of the time card. The time card needs to be kept on site at all times.
4. Time Card Reference Guide – provides helpful information/instructions on filling out the time card.
5. Return to Work (RTW) Program Description - provides details on our program and its benefits for nonprofits, employers, and employees.
PLEASE REMEMBER: Volunteers are NOT allowed to:
- Work off site, in parking lots, or use personal or company vehicles for any job tasks
- Handle cash transactions
- Work directly with children or animals
- Work with knives or direct heat
- Have access to sensitive information
